Event Day: Demo Model

How to sign up: 

1. During checkout, you'll have the opportunity to apply as a demo model for the event. Note: Submission does not guarantee selection. Demo model sign-up ends October 17th & selected models will be contacted October 18th.
2. Indicate which model slot you would like. Check below for model times.
3. If selected "Yes", please select your treatment preference: XERF or Ultra Laser. Treatment preference does not guarantee selection.

Terms & Conditions:

Note: Terms & Conditions & consent forms (below) will be sent out through email prior to the event day to be signed & emailed back. 

As a demo model, you understand the following:

• I consent to being watched live by attendees during my demo treatment.
• I consent to photo/video recording & for Refine Medispa to use the media for marketing, education & internal training. 
• I confirm I will complete the required medical intake and consent forms & follow staff instructions for eligibility & safety.
• The demo is complimentary (up to 150 shots, valued at $1000)
• Only 3 demo spots are available; submission does not guarantee selection 
• I will complete a medical intake and understand staff may decline treatment if not suitable. 
• I release Refine Medispa from liability for normal sides effects (redness, swelling, tenderness, bruising).
• If selected as a demo model, your ticket will be refunded after the spa confirms & you accept the model terms. 
• Ultra Laser: (choose neck or hands) targets rough texture, pigmentation spots, tone, melasma & enlarged pores
• XERF (jawline lifting - half side): targets sagging skin, crepe-like skin, weak collagen, droopy eyes, skin firming, texture & youthful glow.

Demo Model Times:

1:30-2:30PM : XERF model (Please select 1-2pm slot.)

4:30-5:30PM: Ultra model (Please select 4-5pm slot.)

6:30-7:30PM: XERF model (Please select 6-7pm slot.)
